The lungs of birds are relatively small, but are connected to eight or nine air sacs that extend through much of the body, and are in turn connected to air spaces within the bones. On inhalation, air travels through the trachea of a bird into the air sacs. Air then travels continuously from the air sacs at the back, through the lungs, which are relatively fixed in size, to the air sacs at the front. From here, the air is exhaled. These fixed size lungs are called "circulatory lungs", as distinct from the "bellows-type lungs" found in most other animals. The lungs of birds contain millions of tiny parallel passages called parabronchi. Small sacs called atria radiate from the walls of the tiny passages; these, like the alveoli in other lungs, are the site of gas exchange by simple diffusion. The blood flow around the parabronchi and their atria forms a cross-current process of gas exchange (see diagram on the right). == Structure == Aggrecanase is a very large molecule. Its size is estimated to be around 2.5 million Daltons. The molecule is built from a core protein that weighs about 250 kilodaltons. The molecule is made of multiple subunits, each with a specific function. It has a signal sequence of about 170 amino acids in length. The purpose of this sequence is to direct the protein to the correct location of the cell. Another subunit is the pre domain, the metalloproteinase domain, which is around 230 amino acids in length. Its purpose is to keep the enzyme inactive until necessary. Another way the enzyme will remain inactive is in the absence of its catalytic region, which is dependent on zinc ion. Its purpose is to help bind the molecule and stabilize interactions. Attached to the core protein are glycosaminoglycans, a long chain of negatively charged polysaccharides. These glycosaminoglycans consist of chondroitin sulfate and keratan sulfate. The glycosaminoglycans being negatively charged, allow the aggrecan to hold large amounts of water. This is beneficial to the molecule because aggrecanase is a part of connective tissue, cartilage. The water allows the cartilage to better resist pressure when the joints it protects are compressed. This is because the water does not allow it to collapse, but instead spring back and maintain its structure.

Christianity and Druze are Abrahamic religions that share a historical traditional connection with some major theological differences. The two faiths share a common place of origin in the Middle East and are both monotheistic. Christian and Druze communities share a long history of interaction dating back roughly a millennium, particularly in Mount Lebanon. Over the centuries, they have interacted and lived together peacefully, sharing common social and cultural landscapes, despite occasional exceptions. Moreover, Druze beliefs, scriptures and teachings incorporate several elements from Christianity. Historically, the relationship between the Druze and Christians has been characterized by harmony and peaceful coexistence. The divisions within society mainly ran along the lines of class distinction between elites and commoners (alahi) instead of along religious divides in places like Mount Lebanon. Amicable relations between the two groups prevailed throughout history, with the exception of some periods, including 1860 Mount Lebanon civil war. In the Levant region, the conversion of Druze to Christianity was a common practice. Throughout history, there have been instances where prominent members of the Druze community, including some of Shihab dynasty members, as well as the Abi-Lamma clan, embraced Christianity. To reduce the cost to the military of maintaining stockpiles of certain pharmaceuticals, the United States Department of Defense (DoD) and the Food and Drug Administration (FDA) operate a joint initiative known as the Shelf Life Protection Program (SLEP), which evaluates the long-term effectiveness of medications stockpiled by the DoD and other government agencies. Under the program, medications are tested for safety and stability for extended periods of time in controlled storage conditions. In many cases, medications tested were found to be effective for years past their printed expiry dates; a 2006 study by the Journal of Pharmaceutical Sciences found that two-thirds of 122 medications tested through SLEP remained effective for an average of at least four additional years. In 2016, the DoD reported that the program had helped save the department US$2.1 billion on replacing stockpiled medications. While testing and stability assessment found that 88% of the lots were effective at least one year after their expiration date, with an average extension of 66 months, the additional stability period was very variable. For antibiotics studied, the mean extension for ciprofloxacin was 55 months, and for amoxicillin 23 months.
=== Spectral skewing === Spectral skewing is the change in relative intensity of mass spectral peaks due to the changes in concentration of the analyte in the ion source as the mass spectrum is scanned. This situation occurs routinely as chromatographic components elute into a continuous ion source. Spectral skewing is not observed in ion trap (quadrupole (this has been seen also in QMS) or magnetic) or time-of-flight (TOF) mass analyzers because potentially all ions formed in operational cycle (a snapshot in time) of the instrument are available for detection.
Sorafenib's inhibition of UGT1A9 and UGT1A1 may increase plasma concentration of other drugs. The same goes for the CYP2B6 and CYP2C8 pathways, they are inhibited by sorafenib. Giving sorafenib in combination with rifampicin or inducers of CYP3A4 can decrease plasma concentration of sorafenib. CYP3A4 inhibitors are unlikely to affect sorafenib. Sorafenib is a competitive inhibitor of CYP2C19, CYP2D6 and CYP3A4. It inhibits P-glycoprotein, therefore it can increase the plasma concentration of drugs which are P-glycoprotein substrates. Pazopanib is metabolized in the liver by CYP3A4 enzyme. Strong CYP3A4 inhibitors, other than pazopanib, can increase the plasma concentration of pazopanib, and CYP3A4 inducers will do the opposite. Grapefruit juice is a CYP3A4 inhibitor and should be avoided when taking pazopanib. It is also a weak inhibitor of other liver enzymes, CYP2C8 and CYP2D6. Axitinib is metabolized by CYP3A4 and UGT1A1. Strong inhibitors of CYP3A4 will increase the plasma concentration of axitinib, while weak inhibitors have less effect on the plasma concentration. Strong inducers of CYP3A4 will decrease the plasma concentration of axitinib and should be avoided.
== Side effects == General side effects that have been reported with enobosarm in clinical trials include headache, fatigue, anemia, nausea, diarrhea, and back pain. Enobosarm has shown dose-related adverse effects on serum lipids, sex hormone and gonadotropin levels, and carrier protein levels in clinical trials. It decreases HDL cholesterol levels, reducing them dose-dependently by 17% at a dose of 1 mg/day and by 27% at a dose of 3 mg/day. Decreases in total cholesterol levels and in triglyceride levels have also been seen, whereas LDL cholesterol levels are unchanged. In healthy elderly men, total testosterone levels decreased significantly at doses of 1 and 3 mg/day (-31% and -57%, respectively), whereas levels of free testosterone, dihydrotestosterone (DHT), estradiol, luteinizing hormone (LH), and follicle-stimulating hormone (FSH) did not change significantly at doses up to 3 mg/day. In healthy postmenopausal women, LH and FSH decreased significantly only at the 3 mg/day dose (-17% and -30%, respectively), whereas levels of total testosterone, free testosterone, DHT, and estradiol did not clearly change relative to placebo. SHBG levels were lowered at doses of 1 to 3 mg/day, decreasing dramatically by 61% in men and by 80% in women at the 3 mg/day dose. For comparison, testosterone enanthate by intramuscular injection at a highly supraphysiological dose of 600 mg/week resulted in only a 31% decrease in SHBG levels. Despite the large changes in SHBG levels, levels of free testosterone did not significantly change in either men or women.

== P2Y and P1 receptors == Both of these metabotropic receptors are distinguished by their reactivity to specific activators. P1 receptors are preferentially activated by adenosine and P2Y receptors are preferentially more activated by ATP. P1 and P2Y receptors are known to be widely distributed in the brain, heart, kidneys, and adipose tissue. Xanthines (e.g. caffeine) specifically block adenosine receptors, and are known to induce a stimulating effect to one's behavior.

Speech is often unclear due to the inability to pronounce certain sounds. One of the surgical treatments for velopalatal insufficiency involves tailoring the tissue from the back of the throat and using it to purposefully cause partial obstruction of the opening of the nasopharynx. This may actually cause OSA syndrome in susceptible individuals, particularly in the days following surgery, when swelling occurs (see below: Special Situation: Anesthesia and surgery). Finally, patients with OSA are at an increased risk of many perioperative complications when they are present for surgery, even if the planned procedure is not on the head and neck. == Cardiology == In cardiology, genetic conditions such as Brugada syndrome can share features with related disorders caused by mutations in the same gene. An overlap syndrome can be seen whereby a mutation in the SCN5A gene encoding the cardiac sodium channel causes a reduction in the peak sodium current leading to the typical ECG features of Brugada syndrome, but which simultaneously increases the sustained late sodium current leading to the ECG features of Long QT syndrome type 3. Brugada syndrome can also overlap with arrhythmogenic cardiomyopathy due to certain mutations in the plakophilin gene.
